ARE YOU AN ELIGIBLE CANDIDATE FOR THE COURSE? Contact your home Parish and schedule an appointment with Pastor, Associate Priest, Deacon or Adult Faith Formation Coordinator for an Interview. Make sure interviewer signs the form.

WHEN DO YOU NEED THIS INTERVIEW COMPLETED? Your interview should be completed no later than the first week of class.

HOW DO YOU KNOW YOU ARE ENROLLED IN THE COURSE? You need to complete the hard copy registration Application as soon as your payment goes through and submit as soon as you are given a google classroom code. Make sure to write your name exactly how it appears on your Baptismal Certificate.
¿CÓMO SABES QUE ESTAS REGISTRADO/A EN CLASE? Necesita completar la Aplicación de registro impresa (de papel) encuanto el pago del curso sea procesado y se te a dado un codigo del salón de google para la entrega. Asegúrese de escribir tu nombre exactamente como aparece en su certificado de Bautismo.
DO I NEED A SPONSOR AT THE CEREMONY? Yes, please download the forms on the following link and give to your Sponsor(Godparent) to complete. The Guidelines per Canon Law are on a handout attached to the link named Sponsor Registration Form. The Sponsor Pastor Statement if for the sponsor to take to his or her home Parish to have it signed.
¿NECESITO UN ESPONSOR EN LA CEREMONIA? Si, por favor baje las formas en el enlace siguiente y entregalas a tu esponsor(padrino/madrina) para completar. Las Directrices del Canon se encuentran adjunto en el formulario de Registración del Esponsor.
